Seafield Services Outpatient
Program consists of a thorough
evaluation.
The
Evaluation Process begins with an assessment and initial
screening to determine if the individual is appropriate for the
level of care provided at a Seafield facility. When a person
is determined to be appropriate for care, he or she will be advised
of assessment findings and recommendations for admissions will
be made. The admissions process will begin, with the intention
of placing the person into treatment within twenty-four hours.
The
Admission Process entails a thorough biopsychosocial history
performed by a qualified healthcare professional. A nursing assessment
is conducted by a RN or NP, with further evaluation completed
by the physician or nurse practitioner, as needed. Mental
status and psychiatric evaluations are conducted by a staff psychiatrist
when indicated.
All admissions are made with the
final approval of the physician. At the time of the admission:
- all necessary releases are obtained;
- the referral source is contacted;
- the person admitted will be assigned a
case manager;
- a formal treatment plan is developed with
the client within the first ten (10) days of treatment.
Treatment plans have the input
of the clinical director, physician and other appropriate staff
members.
Most insurance accepted.
When needed self-pay (sliding scale) arrangements can be
made.
The
Intensive Program component meets for two to three
hours per session up to five (5) times per week. Frequency
and duration will be determined by the treatment team (Seafield
Clinical staff and the referring agency). During the intensive
component, concrete, specific and measurable goals and objectives
are identified and monitored. Family members are always
encouraged to become involved in their loved ones treatment.
Family Bridge Groups and/or family sessions are available
to help the entire family system benefit from every aspect
of the recovery process. In addition to group
process, each person will receive individual counseling
sessions. Upon completion of the Intensive Level of
Care Component, or by recommendation, the client will be
transferred to another level of care that will best address
his/her individual needs.
The Clinic Level component
will last for the remainder of the client's treatment. Each group
meets for 60 minutes. Frequency and duration is based on individual
needs. The program is designed to work on issues of early recovery,
stabilize a sober support system and enhance the client's acceptance
of sober lifestyle. |
