In some cases, a patient's insurance may require pre-certifications prior to admission. We provide assessments & pre-certifications at any of our Seafield outpatient facilities.

FOR PATIENTS:

Please bring the following:

  • Health insurance card 

  • Prescription card & 30 day supply of daily medications.

  • Photo ID

  • A week’s worth of appropriate clothing

  • Money for miscellaneous expenses ($20/$30) for laundry, vending machines, phone calls & detergent.

If you are an OASAS licensed outpatient program, EAP and/or union & determine that a patient meets the inpatient criteria, call us for direct admission.
